Louisville, Kentucky. Brand new Ford minivan (with 7.2 miles on it) that was on top of a large car carrier truck that hit the bottom of a bridge. It happened in upstate New York in the middle of winter. There was so much snow on the freeway that they couldn't plow it down to the asphalt which obviously caused the truck to be up several inches higher than normal and therefore hit the bottom of an underpass.
